Notice of Privacy Practices
This notice explains how we may use and share information about your child’s health, and the rights you have over that information.
What this notice is
A federal law called HIPAA requires every health care practice to give patients a written notice describing how it handles health information. That notice is called a Notice of Privacy Practices, and it covers things like:
- How we use your child’s information to provide care, bill insurance, and run the practice.
- When we’re allowed to share it, and when we need your permission.
- Your rights — including the right to see your child’s record, ask for a copy, ask us to correct it, and ask for a list of who we’ve shared it with.
- Who to contact if you think your privacy has not been protected.
A paper copy of this notice is also available in our office. Ask at the front desk and we’ll hand you one — you don’t have to explain why.
