Notice of Nondiscrimination
Every family is welcome here. This notice explains that promise and how to speak up if you believe we fell short of it.
What this notice is
Section 1557 of the Affordable Care Act is the part of federal law that says health care providers cannot treat people differently because of race, color, national origin, age, disability, or sex. Practices that receive federal funds are required to post a notice saying so.
The notice has to be conspicuously posted — meaning it must be easy to find, not buried. That includes the practice’s website, the office itself, and significant communications with patients.
A practice with 15 or more employees also has to name a person responsible for handling complaints and explain how to file a grievance.

How to file a grievance
